چکیده

Background Lateral wedge insoles have consistently shown to reduce the external knee adduction moment (EKAM) in medial knee osteoarthritis (OA) patients; although there is evidence that certain patients have a paradoxical increase in EKAM. This may be a key factor in determining clinical response and thus identifying and understanding why these patients increase EKAM is critical for prescribing the correct treatment for these patients. Previous evidence has suggested that foot and ankle biomechanics play a role in reducing EKAM by shifting the centre of foot pressure (COFP) laterally and increasing the valgus orientation of the calcaneus, which shortens the lever arm in respect of the knee, thus reducing the EKAM. To date, patients have been studied irrespective of biomechanical response to lateral wedge insoles. In this study we investigated whether dynamic ankle biomechanics can assist in identifying and explaining why some patients increase EKAM and other decrease EKAM when wearing a lateral wedge.

Ankle motion influences the external knee adduction moment and may predict who will respond to lateral wedge insoles?: an ancillary analysis from the SILK trial

OBJECTIVE Lateral wedge insoles are a potential simple treatment for medial knee osteoarthritis (OA) patients by reducing the external knee adduction moment (EKAM). However in some patients, an increase in their EKAM is seen. Understanding the role of the ankle joint complex in the response to lateral wedge insoles is critical in understanding and potentially identifying why some patients respo...

Motion of the rearfoot, ankle and subtalar joints and ankle moments when wearing lateral wedge insoles – results from bone anchored markers

Background Knee osteoarthritis is a debilitating condition and increased dynamic loading at the knee has been linked with increased progression of the disease. Lateral wedge insoles have been used in clinical practice since the late 1980s. It is theorised that lateral wedge insoles increase the subtalar joint valgus orientation and increase the ankle valgus moment [1], with subsequent reduced k...

Biomechanical effects of lateral and medial wedge insoles on unilateral weight bearing

[Purpose] Lateral wedge insoles reduce the peak external knee adduction moment and are advocated for patients with knee osteoarthritis. However, some patients demonstrate adverse biomechanical effects with treatment. In this study, we examined the immediate effects of lateral and medial wedge insoles under unilateral weight bearing. [Subjects and Methods] Thirty healthy young adults participate...

Lateral wedges with and without custom arch support for people with medial knee osteoarthritis and pronated feet: an exploratory randomized crossover study

BACKGROUND Pronated foot posture is associated with many clinical and biomechanical outcomes unique to medial compartment knee osteoarthritis (OA). Though shoe-worn insole treatment, including lateral wedges, is commonly studied in this patient population, their effects on the specific subgroup of people with medial knee OA and concomitant pronated feet are unknown. The purpose of this study wa...

Biomechanical Analysis of the Influence of SACH Foot and Dynamic-Response Foot in Individual With Unilateral Transtibial Amputee During Running

Objective: Amputation of the lower limb due to loss of part of the musculoskeletal structure reduces performance and increases injury during locomotion. The effect of various types of prosthetic feet has been analyzed in several studies during running. The purpose of this study was a biomechanical analysis of the influence of SACH and Dynamic-Response foot on several kinetic variables in the st...
